The main mechanisms for the development of chronic fatigue are:

  1. Hormonal imbalance - excess cortisol causes:

    • Increased muscle tone
    • Disturbance of microcirculation

  2. Neurological disorders:

    • Hypertonus of neck muscles - cervical scoliosis
    • Spasm of the diaphragm - respiratory disturbance
    • Compression of cervical nerve roots - headaches.
  3. Biomechanical changes:

‘I was living like a fog...’

‘After my divorce and problems at work, I was literally falling apart. Constant migraines, intestines spasmed to the point where eating was impossible, and fatigue.... I woke up already broken. The slightest movement required incredible effort. My neck wouldn't turn, my back hurt all the time.

I went through all the doctors - gastroenterologist said I had IBS, neurologist said I had BPD, chiropractors temporarily levelled my pelvis, but after a week everything came back. Antidepressants, antispasmodics, massages - nothing had a sustainable effect.

When I came to Paupio Kinezioteka Clinic, I had:

  • 50% limitation of head rotation

FSS Chronic Fatigue Scale

The Stanford University Fatigue Scale (FSS) is an instrument used to assess the level of fatigue in patients.

It consists of 9 questions, each of which is scored on a 7-point scale from 1 to 7, where 1 means ‘never’ and 7 means ‘always’.

9 questions

  1. My motivation decreases due to fatigue.
  2. Physical exertion makes me very tired.
  3. I get tired easily even after small efforts.
  4. Fatigue prevents me from performing work or home duties.
  5. Fatigue makes it difficult for me to concentrate.
  6. Fatigue interferes with my social life.
  7. Even after resting, I feel exhausted(s).
  8. My fatigue prevents me from doing sports or physical activity.
  9. Fatigue is one of my biggest problems.

The total score is calculated by adding the scores for all questions and dividing by the number of questions (9).

The FSS scale is widely used in clinical trials to assess treatment efficacy and changes in patients' fatigue levels.

  • <36 points - normal or mild fatigue.
  • 36-52 points - moderate chronic fatigue.
  • ≥53 points - severe

Comprehensive diagnosis and explanation of the causes of chronic fatigue

When our patient first crossed the threshold of the centre, we saw a whole complex of interrelated problems. Her body was literally crying out for help - stiff neck, twisted pelvis, shallow breathing. A detailed examination revealed three key areas of disturbance:

Muscular imbalance was immediately apparent - an overstretched sternoclavicular muscle on the right was literally pulling her head to the side, while weakened gluteal muscles on the left could not hold her pelvis in the correct position. And the iliopsoas muscle, spasmed from constant stress, exacerbated this misalignment.

The respiratory system__ was working half-heartedly - the diaphragm, our main respiratory ‘pump,’ was moving with an amplitude of only 40% of normal. Instead of physiological diaphragmatic breathing, the patient was using superficial upper thoracic breathing, further increasing tension in the neck and shoulder girdle.

The Biomechanical abnormalities formed a clear picture: functional shortening of the right leg by 1.5 cm, block in the occipito-atlantar joint (C0-C1) and pronounced counterclockwise rotation of the pelvis. All this created a vicious circle of pain and tension.

Treatment strategy: three stages of recovery

Breaking the vicious cycle (first 6 weeks):

  • Relieve hypertonicity of the neck and lower back muscles with cryotherapy and myofascial release
  • Restore diaphragm mobility through breathing practices
  • Eliminate pain syndrome with decompression hangs on the Speedience simulator.
  • Begin pelvic alignment with gentle isometric exercises.

Rebuild the system (8 weeks of intensive work):

  • Form a new motor stereotype through kinesiotherapy complexes
  • Strengthen the muscular corset with emphasis on gluteal and deep abdominal muscles.
  • Introduce asymmetrical loads to correct pelvic misalignment
  • Connect neuromuscular stimulation of problem areas

Fix the result (6 weeks of stabilisation):

Main stages of the programme for the treatment of chronic fatigue and the effects of stress

Our centre uses a three-module recovery programme for comprehensive treatment. (Each module: 5-7 weeks, cycle ‘doctor's appointment - 6 sessions - control doctor's appointment - 6 sessions - final doctor's appointment’).

We adjusted the programme for our patient taking into account her individual peculiarities and made changes 2 more times at the end of the sessions in the centre. As a result, the patient followed the following programme:

Module 1: Decompression and relief of acute symptoms (6 weeks).

Goal

To eliminate muscle hypertonicity, restore breathing, relieve pain.
